Room Reservation

USO Fort Campbell Room Request Form

All patrons requesting a room reservation for an event agree to read and adhere to the USO Room Usage Agreement guidelines below.

It is the responsibility of the undersigned to check in (with a military ID) and out with the USO Staff prior to and before leaving the event to ensure cleanliness or assess any damages to furniture, equipment, floors or walls. Conference Room/Event Center Sign In/Sign Out checklist will be followed.

It is the responsibility of the undersigned to ensure that policies are followed, the requested room is returned to the layout designated by USO Staff, tables and chairs properly wiped clean, floors swept/vacuumed and mopped, and supplies and/or trash is removed. USO Volunteers are not required to perform this for your event.

A limited number of tables and chairs are available for use at the USO for the Event Center. Patrons are responsible for setting up their own event. There are no kitchen facilities available at the USO Event Center/Conference Room.

The Conference Room/Event Center may be reserved during operating hours: Monday-Friday 0800-1900 and Saturday and Sunday 1000-1500. (Depending on staff availability)

The Conference room can sit up to 30 guests. The Event Center can fit up to 250 guests.

Alcoholic beverages and tobacco products are prohibited in the facility.
Smoking or vaping is not allowed inside the facility, and outside must be conducted at least 50 feet from the building.

TV in the Conference Room and A/V equipment in the Event Center shall only be used if agreed upon prior to event.

Parents/Guardians are responsible for their children at all times. Children must remain with their Parent/Guardian at all times inside the USO.

It is the responsibility of the undersigned to abide by all Center Operation Rules as directed by USO Staff and Volunteers.

It is the responsibility of the undersigned to provide a head count to USO Staff at the conclusion of your event.

If you would like to provide refreshments for your event you are welcome to bring them into the facility.

Groups using USO facilities are not permitted to sell items, take donations, or fundraise. Groups will not sell tickets to an event being held at USO facilities.

Failure to comply with the terms of the agreement or a “No Show“ will result in a suspension of privileges to reserve any USO Fort Campbell space for the requesting company for a period of 3 months.
